One Voice for Volusia is proud to share the collaborative Community Health Needs Assessments (CHNA’s) for Flagler and Volusia Counties. Through partnerships with Flagler Cares, AdventHealth, Halifax Health and the Departments of Health in Flagler and Volusia Counties, these landmark collaborative assessments were conducted and designed to identify health issues of primary concern for each county and to provide critical information to those in a position to achieve optimal health for their citizens and consumers.
2023-2025 Community Health Improvement Plan (CHIP)
Once a Community Health Assessment is complete, the stakeholders meet to develop an improvement plan to move forward actionable items identified in the Community Health Assessment.
